Who should use this form?
State associations or chapters that need proof of insurance for an event, conference, or venue should complete the Certificate of Insurance Request Form. Please ensure the request comes from an authorized state leader or adviser.
How to submit
Fill out the online request form here: COI Request Form. Be sure to provide complete and accurate details, including the name and address of the certificate holder, event information, and any special wording required.
Processing time
Requests take up to 10 business days to process. Please plan ahead and submit early to ensure the COI is ready in time for your event.
